Summary of Position
This position is responsible for providing patients with assistance in meeting their healthcare needs including enrollment in Medicaid Expansion application assistance, pharmaceutical charity programs, Medicare, ACA Health Insurance, preparing disability applications for patients through the SOAR program, providing education and completing paperwork for sliding scale and new uninsured patients, Medicaid Expansion application assistance, and referring patients to other community resources based on their expressed needs. Address all patient referrals through the Provider Referral Work Que. Coordinate outreach/in reach events during National Health Center Week and beyond, create and maintain relationships with community agencies and elected officials. Equitably serve the organization’s patient population through patient advocacy, community, outreach, and engagement.
Minimum Qualifications
The following qualifications are the minimum necessary to adequately perform this job. However, any equivalent combination of experience, education, and training which provides the necessary knowledge, skills, and abilities would be acceptable, subject to any legal and/or regulatory requirements; Must be able to sit, stand and walk for long periods of time; Ability to read and understand the English language; Ability to effectively maintain confidentiality of records and communicate with all levels of personnel.
Education: High School Diploma or GED certificate required; bachelor’s degree preferred
Certification(s): SHIIP Counselor, Affordable Care Act (navigator or certified application counselor), SOAR Counselor, ACA Navigator, completion of TPC training, Community Health Worker Certification
